Something like this happened to me when I was playing 
Ezreal on a friend's account. I was being chased by Diana (We both had pretty low health at the time), when I saw that my ultimate was up. I said "Screw it" and shot it at her. It literally did no damage to her, but I managed to kill the also low health Vladimir behind her. Even the Diana player was like "How did that do no damage?", because I hit her dead center.
We tried to recreate it, but with no success. Must have been the skins or something, but even that didn't help in recreating it.
