    How/Where can I play team builder game?

    I just want to get a summoner icon by playing 5 games with one champion, but I can't find this game mode anywhere. Is it still available or is there any way to get free summoner icons?

    No, the team builder icons are not available and haven't been for 2 years. So unfortunately you can not unlock them unless Riot brings them back with missions at some point.
