Regarding Gnar, he's hard but you can deal with it cause at some point you'll just get too tanky and he won't be able to kill you.
Regarding AP tops, the best way to deal with them is to not pick rammus against them at all. You cannot get in range to them by any means nor can you make use a powerful armour build because you have to stack so much freaking magic resist to even survive lane.

Do you recommend top rammus to someone trying to switch from adc to top lane with experience mostly with carry top laners like Jax or Irelia?
As mentioned it is a quick guide.
I overall wouldn't recommend Rammus top if you're looking for a carry top laner. I consider Rammus a 'top lane anticarry' meaning he's going to shut down these carry top laners himself - so if you're looking for ways to shut those down when you can't pick them I'd recommend him.
