this guide = ****py damage output
why would you get zhonyas, being able to get away like that isnt worth the inability to do anything until very late game, and even then your damage output wont be nearly as high as it should
the fact that you get IE so late makes you very close to useless
also there is no reason why you shouldnt be maxing out expunge first...
Personally, I think maxing out your stealth first is better. Its how I play Twitch, and it works well for me.
The Hexdrinker is Optional, I just like it when playing a Karthus, but I do suppose this is not a place for personal preferences.

Let me straighten you out good sir.
Zhonya's = bad
Hexdrinker = okay if your facing 5 mages
Wit's end = normally good, bad on Twitch
So. I would suggest Guardian Angel or Randuin's instead of Zhonya's, Last Whisper instead of Hexdrinker, YoYo's or something else instead of Wit's End.
Your runes are ****ed up as you won't be doing magic damage with this build, I suspect you want ArP marks, Armor Seals, CDR/AS glyphs and AD quints.
