#1: You're not going out of ELO hell. Since the moment you wrote it, you're bound.

#2: Answering your question:

Tanky-AP Morde (as the name suggests, the tank items come first)
Sona or Soraka if you like support
Alistar or Amumu -> tank
Trynda or Ashe -> AD
Master Yi or Jax -> backdoor

idk about a good AP, I hardly play as them. And do not get Vayne. She needs a good start and a good team to work well, and I doubt if you'll find any good human to play with.

Oriana, Annie, brand, morganna for mid AP.
Irrelia, Singed and Jax for top.
Nocturne, Gangplank, Lee sin, Ammmumu and Fiddle for jungle.
Alistar, Taric, Sona and Janna for support.
Vayne, Ashe, Corki and Trist for bot AP.
These are the solo queue stomp champs atm at my ELO and higher(I watch a lot of streams :))

Getting out of Elo Hell is mostly about yourself. Everyone has his own Elo Hell or Elo Hells. Elo hell means the range of Elo you can't get out from. While some people's Elo Hell is at 900-1200 others' might be at 1600-1700. What you gotta do to get out? Try becoming the so said leader of the team. Don't rage, be calm, try to motivate them. If they flame you don't reply with flaming. Most importantly place team achievements above personal achievements. Who the **** loves supports that take all the kills from the AD carry? etc.

To be honnest, from 900 to 1200, you can basicaly play any champion you want. I did in the last few weeks. To make a short story, I started playing ranked short after I hit lvl 30, lost tons of games, tried again a few months later and fell to 900. Then I played arround 600 more games. And won 90% matches from 900 to 1257 (actual ELO) using whathever the champion.
-Wards
-General map awareness
-Know when to dive and when to fall back
-Know if you will be able to get that inhib or if you should baron instead after an ace
-etc, etc

is what will get you out of ELO "hell" (btw, no such thing as ELO hell, I was in there because I was more or less as bad as my teamates.)

To some extend, humility is what will "carry you out of ELO hell"...

Yeah okay there are some exceptions. But beside Twitch/Eve, most champs will do just fine.

A great team setup will require a Tank, ranged AP Carry, Ranged AD Carry, Jungler, and a support.
An example of a good team setup:
Alistar
Fiddlesticks
Corki
Nidalee
Xin Zhao

Alistar and his great CC affects will pick up kills easily if done right. He is also one of the hardest tanks to kill because of his ultimate.
Fiddlesticks can be your jungler and his fear is very great on the enemy' carry. His ult has also a great AOE affect so it hits everyone in team fights.
Corki will deal tons of damage and can chase enemies down easily.
Nidalee heals tons of HP and has great map awareness with her traps.
Xin Zhao can also be your jungler and he will rip the enemy's tanks apart with his ultimate.

This team composition has great CC, great support and able to deal tons of damage.
