Are we really going to get into a discussion about Blood-mages? Riot clearly themed him to represent Vladamir The Impailer. You know the notorious ruler back during the Byzentine empire (im not 100% sure on that tbh). He's thought to be the inspiration for the myth that now thrives today as "Vampires".
We can get into a big deep discussion about this, or you can read his back stroy and realize when they mention Noxian forces going missing near his castle it's because Riot wants us to believe him to be a vampire. Even his mechanincs make him vampiresque.
So....I guess we can comprimise he's a Vampiric Blood Mage. /thread
Oh and let's not forget his Nosferatu skin....Sure it was Halloween last year, but still. It's a BIG hint.....

I thought the fact that he liked to stick conquered peoples on stakes....10 foot tall stakes, and let gravity impale them was quite interesting.....Or that for torture tactics he'd slice someone open and pull their lungs out through their chest making them look like an eagle spreading its wings was pretty interesting too.
Yea.. but most people know that stuff about him.. He at one point basically made a "forest" of impaled soldiers to send fear into his enemies.. Fun stuff or for example he liked to dip his bread into blood while he ate it.

I read a history book about him in my history class. In it, it was said that his bedroom was right above the torture chamber, so he could always hear the screams of agonie of his victims. This gives you an idea on how brutal and sadic that guy was. No wonder people started to make legends about him being a demon/vampire
