Same thing happens no matter what role/champion I play.
This is the negativity I mean.
GET RID OF IT! NOBODY is innately bad. EVERYBODY has bad games/streaks of bad games.

It's hard to be positive when you can't go positive.
And you'll never GO positive if you're not positive.
See the vicious cycle you got yourself in?

I'm loving your B Positive attitude, Xenasis.
-Terrible Vlad joke.

Seriously though - you are a REALLY good player. You get yourself down which lets your performance down which gets yourself down even more.
I don't think that's why though. I don't know what it is, I just lose every lane. No matter how confident I feel.
I mean, how can I ever play ranked like this? I can't play in any lane and I can't jungle, lol.

You don't need to ever play ranked. I don't. As stonewall put it, unless they hand out a free trundle skin for a certain elo you gain nothing from it so I won't be doing it.
It's ironic you lose confidence by saying you're confident.
Instead of "I CAN'T PLAY X LANE", think "I did badly that time, but next time, I can X, Y, and Z!".
Believe me. You're not confident. You need confidence. Real confidence, not faux confidence.

My CS was almost perfect actually, at least better than my usual standards. Then she got Phage and Sheen and started to zone us back to tower.
Your usual standards are low because you play so little, then.
I remember Blitz saying she had 40 more cs than you when you died at one point.
Don't be so down on yourself all the time. If you always pass things off as being "I SO BAD" all the time. If you do that you'll never improve. Mental state is the key to winning most games. Fact.
