Engaging as Tryndamere against a Karthus with his ultimate up in a teamfight can be risky. However, as a hard carry, your job is not to die early game so you can farm up late game, where you'll be able to just spin in and kill that Karthus incredibly easily. However, his passive makes this an issue, as it's still possible for him to pop Requiem during those 8 seconds. In team fights, if you can kill Karthus and still manage to get out safely without popping your ultimate, you should be fine. Or just prioritize Karthus as a team focus so that you can pop your Undying Rage without fear. Also, if you get some magic resist items, you can heal enough with your 8 stacks from Bloodlust at level 18 so that you can survive Requiem after he bombs it.

If it is ranked, and you see a Karthus, get a Heal on him.
Investment of 2xVamp scepter is a fairly runaway from his ulti on low HP if mobs are around. 1 crit is usualy enaugh with 2x vamps to stay alive.
