ok here it is, 30 seconds long but just to point out from my own view
1. I do have about 1/5th of my Health when I activated Blade Waltz
2. You can clearly see Blade Waltz must have been hit because it goes on cooldown
3. I watched three times and I don't see any characters use any non targeting abilities around me.
So I will let you guys judge

A bomb was placed on your head by Zilean which counts as "overtime effect". Besides that, Ryze shot some auto attacks at you, that have a delay that is long enough to reach you when you are already in your ult.

I think you got killed during the channel cast time of the ultimate, or by the slower animation on Ryze Q's as banana mentioned.
A bomb was placed on your head by Zilean which counts as "overtime effect". Besides that, Ryze shot some auto attacks at you, that have a delay that is long enough to reach you when you are already in your ult.

The zilean bomb exploded after his death, so it definitely wasn't zile.
Im quite sure it was not a bug. Perhaps that it was due to the little bit of server delay.
