She has a fair amount of counters, late-game she dies pretty much instantly and CC stops her in her tracks. She hardly sees tournament play because of all this. She doesn't need nerfs, maybe some fixes because the remove-cast-time tricks are stupid.

Countering her in lane mostly depends on your pick:
Renekton and
Garen would be the main two candidates I can think of right now. A lot of champs will also be more useful than her in teamfights if she doesn't snowball, so playing it safe is generally a pretty good idea.
In teamfights the best way to deal with her is locking her down with hard CC and bursting her; silences are really good against her too.

Using a ranged champion to harass her a lot at level 1 is effective.
Renekton from my experience dominates her.
If a
Riven knows what she is doing she can all in
Darius at lvl 1,2, or 3 and probably win.
Shyvana is a really good pick against melee, good suggestion. Also
Dr. Mundo has been pretty popular top, he can probably kite riven all day with cleavers

Riven is a champion that to many will look very OP due to how hard she snowballs if she gets ahead. However, she does have quite a few weaknesses which is why I believe she isn't getting nerfed (or at least not yet). Most of her weaknesses have already been mentioned.
She depends on having a strong laning phase to have a strong impact on the game and there are several champions that can shut her down in lane or at least stop her from going out of control (most notable examples are
Renekton,
Garen,
Shyvana or some slightly more niche picks such as
Trundle and
Quinn)
She's also a weak teamfighter against CC heavy teams because she relies on using all of her abilities in a fight to do damage. If you CC her she won't be able to use them and she'll just die due to how squishy she is.
People are quick to say she's OP because if she does get ahead she does absurd amounts of damage, though that's really all she does.
