Your build is weak and so is karthus mid he is better suited top or jung, I started playing karthus after reading this, and was playing with this build and was doing ok around 5-10 kills, but building ap on karthus is unnecessary Will of ancients increases your survivability and allows you to 1v1 all tanks and easily win with lindrys and Archangels staff being the only ap items built. after switching to Spell-vamp/tank karthus I started averaging 20 kill matches, I found that building spirit visage and thornmail makes him viable as an off tank and makes him a very strong engager, as well as giving him good cut off ability, boots are unnecessary as you are either going to get the kill and walk away or die and get the kill and chasing with karth is a joke anyway that is what R is for. W early on their disengage and you should be able to pick up the kill with the q or at least get them low enough for R to pick it up. I found that karthus top has a very strong laning phase and is easily able to zone out early due to the large prevalence of melee champs up top. karthus is just about every melee champs worst nightmare, they can't attack you without taking hard and fast dmg and you no longer have to skill shot, you just drop q on yourself. mid is a poor place for karthus due to large prevalence of glass cannon range chars that can take advantage of aoe border and low hp, they can quick kill you and run while karthus is best at supplying sustained dmg and self healing and then trapping in to pick up the kill

BTW, as for the guide. The spells and items section is not very nice looking :p
Oh wait, did you mention that Karthus is bad at laning but it is very very good at teamfights? Maybe I didn't notice that :p
