I am taking Rylai's crystal scepter, because your w ability is going to do a little more healing, and picked it because it have a slow to so you open up with e ability, it'll slow all of them, and then the team comes flying it to the fight when they are slowed or you can make the team go first and if the other team is escaping just use e, and then the team can kill them when they are slowed!! :D

And you dont have to build in the same way as i did, you can take them after what the team need fx. if they need Life steal and Attack speed in the 20 minuttes then you can take Zeke's Herald instead of any item you want :D
