Try that you go with a carry together :) Try at the beginning that you dont make useless heals or s.th. wait for your manamune that you have a good mana regen.

Why Nashor's Tooth?- Cause for your Q its important that you have a good AS! So you can heal often you team.

Creeping / Jungling

It would be good for you if you get the mana golem. You have big mana probs. and CD's ask your team for it.
