Yeah try swapping out Wits End with The Black Cleaver the passive goes quite well with Malady's and sell Doran's Blade later on for something like Blood Thirster or Lich Bane. Otherwise, the other items are not bad

But, but, but... Malady, Madreds, Wit's End, all do magic damage, poison is magic damage, blinding dart and mushies are magic damage to...
Either you build teemo ad and give him ad items like Bloodthirster, Black Cleaver, IE, Phantom, or you build teemo ap and give him (previous items) Malady, Madreds, Wit's end, maybe Sorc Boots for more mpen, Abysall Scepter for same reason and for mres, the hourglass for more ap and sum armor and a lil chance for escaping with it's active. And if it is ap, then use mpen marks, ap or ap/level glyphs.
Problem with teemo is his low hp, using hp or hp/level seals and quints will solve this problem a bit, or using frozen hammer like you do.
