His mana problems are obvious and hard to deal with. That's why I've wrote that you HAVE to jungle with Trundle.. he's a sitting duck in laning phase if you have to fight against ranged DPS / harassment heroes. He doesn't get any money or even the chance to harass because of his squishyness. Jungling is his duty. And you have to get both buffs as soon as they're respawning. Otherwise a support char like Soraka could solve some of your mana problems. If you're laning Clarity could make a difference but it's also wasted in my opinion - compared to his worthness with a jungle build (Smite / Ghost). :/
I don't see any other solutions than getting the buffs consistently. Mana Items would weaken his viability as an Offtank / Chasing Melee DPS.
Another huge issue that I deal with is mana issues, especially if you're not jungling or not keeping up with blue. How do you feel on that topic and what do you do to remedy the issue?
Now for your items I would suggest rushing Phage > Sheen as Phage ensures kills due to several small issues with Pillar of Filth (as amazing as it is). Early game when you can really use the kills and spare gold (especially first bloods) it's hard to get the kill unless the opposing player is of a lower skill or being stupid as it tends to be necessary to get into melee range and stay there using Pillar of Filth + Contaminate.
You may want to put on some screenshots/videos of managing Pillar of Filth to block/trap heroes and proper usage of Contaminate. I find a lot of Trundle players drop Contaminate right on top of themselves instead of properly ahead or in a location allowing for it's best usage. I know a lot of other guides have done it but it helps out.
