Learn to last hit passively, do NOT fight the enemy if you know your early game is weaker. That's like vayne trying to 1v1 graves at level 2. You'll feed them and piss off the team. Just spend your time farming in your lane while avoiding your opponent the best you can.
Have your Support ward properly but if they don't buy wards on their way back one time and you want wards, buy a ward, just because it's the supports job doesn't mean you can't help, they need some money too.
Learn positioning, you may be equipped to face-melt some noobs but you aren't equipped to take 5 champions at once. Stay in the back, hit whatever target is closest and safest. You aren't the squishy melter, you hit who you can hit without getting hit back. Example, tristana is in the back, tank in the front. Going past the tank just makes it easier for your opponent to kill you, stay behind your tank and let them protect you, its what they're supposed to do. In combat you are the sniper, you want to remain undetected by being on the outskirts of the fight.
Don't get caught into the idea that you can't build situational. You get an IE, you get a PD, you get your BT, but if they have zilcho for armor and you're already melting them, you MIGHT not need last whisper right away. In that game pick up a QSS, or something first, because you honestly don't need that extra Armpen as much as survivability might help.
Learn your opponents, if Malz ults you and you don't have quick silver sash you might be sad. If the enemy has an akali stay where she can't get to you as long as you can, or she'll burst you down and make you worthless.
Don't collect buffs alone. THIS IS DANGEROUS.
this is more of a general tip, get the useable stat potions after full build. they make it that much easier to get by.

Wow, that's a heck lot of information, kinda explains why I get bursted down very quickly, but one little question, what if the tank is
Malphite or some other super tanky champion, who else do I focus then? (if the tank is the only one that's closest and safest I can find) Also, what if they just run at me using stuns and snares?

Focus his *** anyway. If people run at you, kite them as best you can and hope your team isn't useless.

You can make an angle by thinking about your positioning coming into the fight and create an opening to hit another target.
A lot of the time though this isn't very easy and it's just safer to focus them and kite. Ideally someone on your team is going to be trying to protect you.

If they like to flash malphite ult you or flash, double lb w or something, stay out of the teamfight for a bit. Someone like Ezreal can also easily clean up fights just by staying out of the initial engage.
farm + positioning = win.
