I usually get Madred's by 20-22 minutes on an average game, Also, once I start getting that many kills, I usually get at least 1 kill in every subsequent fight, sometimes even if it's a 3 man gank (I'll take out at least one). Basically, I start out kind of slow unless the opponent is just STUP"
