I don't smartcast items I manually click after using the side buttons of my Razer Naga, if you are having trouble with smartcast I would suggest leaving items to manual cast.

"IMO top is way easier than mid as you don't have to contend with assassins and it is more farm based and involves less roaming usually, my opinion though."
