They allow you to do a ton of things, only falling behind laptops on the gaming area, but desktop>laptop in that area anyways.

I bind my 2 thumb buttons in every game I can. Especially FPS games, cuz that's where I feel they help the most.
I use button combinations for my Winamp. Shift+Ctrl+left/down/right/up/PgDn/PgUp for back/play&pause/next/stop/volume up/volume down."
