Generally there is no way for you to win games just by reading what I say though. After you play Yorick enough you'll figure things out for yourself and become really good at the mechanics, to point where you don't need your team.
If your team is doing really bad all game then you have two options. You can either push top extremely hard and kill the enemies if they come to stop you (or run if you can't do that). Or you can use teleport to help your team win crucial fights and then go right back to pushing. When you split push as Yorick it forces the enemy to send 3 or more people to stop you in most cases due to how strong Yorick is. This means your team now has to deal with way less enemies and should be focusing objectives, if they aren't then all you can do is repeat this process until they do. Eventually your team is going to do something and the whole time you're pushing you should be getting towers out of it.
If split pushing really isn't working for you then I'd recommend the new Team Fight build I put up so you can be even stronger in team fights.
