I just play as Jerry (the mouse from Tom and Jerry) and always roll with exhaust and flash. I make sure they never get on my booty. It's really a cat and mouse game. Kinda like being an assassin, but instead of finding opportunities/moments to execute your enemies you dip in and out of situations to help your teammates with phat heals, silences and damage reductions.
I always say, whatever works best for you, roll with that. Forcing someone to play a certain playstyle doesn't work in their favour. It's just what I found myself working the best for me.

How was Mordekaiser's room like, was it messy? Filled with metal band posters and electric guitars? Jokes aside XD Good job on escaping the Death Realm! He should've just went with Morellonomicon.. or Thornmail if your team was heavy on physical dmg.
All you gotta know is that the GW items apply 40% healing reduction. Both Chemtech Putrifier and Morellonomicon applies GW by dealing magic to the enemy champions, whilst Thornmail applies GW when champions auto attack the player with Thornmail.
All three of them can also increase the effect to 60%. For Chemtech Putrifier it's healing or shielding allies. For Morellonomicon they gotta be under 50% of their max hp. For Thornmail your enemies got to be immobilized.
There's not much you can do against anti-heal, it's a counter build item. Things you can do however is dodging abilities so that GW doesn't apply on you. That way atleast you still get full benefit out of the heals from landing Q's and using your R.
EDIT: As of patch 11.18 when using R, Grievous Wounds gets removed from all targets before applying the ultimate heal.

In my opinion Soraka is pretty good in alot of match ups because she brings loads of healing/sustain which is always appreciated by teammates. Soraka's HP is pretty valuable, she sacrifices less HP than the amount she heals her allies, therefore you need to be extra careful and avoid poke as much as you can. When mentioning stuff like Karma's W (675 range) or Ignite (600 range) I start wondering if you're not playing too aggressive/too much forward. Soraka's Starcall has a range of 800 and 235 radius (for a total of 1035 max range), Equinox has a range of 925, yes Starcall travels slower at longer range, but you also need to play at a safe range to not get all-in'd with poke champions like Karma or supports that take ignite. When you survive early laning phase ignite will start falling off and exhaust becomes way better, Soraka usually shines in teamfights where she can heal everyone around her and Q enemies to sustain up. You can't always out push the enemies bot lane, but you can help your adc freeze the minion wave in front of your turret using your Q's or try poking enemies from long range to prevent from being all-in'd.
Locket is a great defensive purchase. Gargoyle Stoneplate however I really dislike the item, at first I was really excited about it but the item only shines when in teamfights, if you're not near 3 enemy champions the item becomes pretty bad, especially because the active is useless considering it doesn't work like a selfheal, the way it works is that it temporarily boosts your CURRENT hp by 40% or 100%, since Soraka always heals her current hp will be rather low and it's temporarily aswell so it'll put your hp back at a lower amount after some secs.
My advice would be to play more passive and stay further back, away from the ignite range and far enough so that you have more time to dodge the skillshots/not be in range of them. As for defensive items you could opt for Locket and/or Zz'Rot Portal, also Seraph's Embrace is quite the defensive item (pretty big shield once you've stacked up Tear). Bottom line, you got to value your own HP as Soraka more than your allies HP (let them take the poke, you can heal them more than you can heal yourself).
Also since I like maxing Q more (as I explained earlier below, more sustain, more damage) Soraka has quite the damage which alot of times is ignored early game and facetanked by enemy champions because they think it won't deal alot of damage.
Overall, playing more passive is safer for yourself and punishing enemies mistakes will be alot more punishing when they greed for damage.
