There are limits to what the body can do to itself in such a state.
If you thought a dragon were to eat you and you were truly suseptible, you'd just go brain dead (after having a horrific experience of being eaten in your little hallucinated dream), cause the brain's perception is "Dawww, I`m soooo dead."
Otherwise, you'd probably wake up later would a ridiculous amount of bruises, torn muscles, and maybe ruptured organs.
Nothing has to attack your skin for it to rip open.
The cells simply let go of the bonds that bind them because of the artificial input by the brain telling them what's going on, and boom, you've got a cut out of no where.
This is also if you're truly hallucinating. Meaning you're under the influence of some foreign power such as a disease, poison, or... Shaco's magic. :P
Thinking you're going to get hurt doesn't cause you to get hurt.
This is all thoroughly established and researched by psychiatrists and psychologists, and is a true phenomenon in real life. :P
