I tried writing a guide something like that previously. Honestly, it doesn't work. The game is too fluid - new picks and bans arise every patch, and even sooner when large-scale games (such as dreamhack) occur. Instead of looking for a guide for picks and bans, browse the LoL forums to see who is considered OP. Alternatively, when Elements gives his meta talks after every patch, he mentions common picks and bans, so look out for those posts.
Right now, a few champs are considered ban-worthy. Annie and gangplank are the only two I can currently recall.

Alot of people has also been banning TF because of his powerful ultimate, but that will probably change with the patch that arrived today..
I'd also say that Amumu and Annie are great bans atm.
Nocturne is also banned quite often in lower elo games, due to his great ganking potential.
