This is a very nice item build. But I would take out Last Whisper for something with some extra crit(just a suggestion) and take out Quicksilver Sash for a BloodThirster. Don't get me wrong, Quicksilver Sash IS nice for it's good MR and debuff, but you won't really need it if you have all of that amazing life steal. Trust me, I've taken on three people at once and won because of it(well, Undying Rage a little bit too:D).

That's a question you should ask Z3rg himself, but my personal take on it:
's Cooldown is reduced by 2 seconds every time you make a critical strike. With that in mind, you can use it for both, in AND out.

Edit: Only tower dive if the enemy is extremely low, and you can kill them with one hit(best for first blood), or if you have your ulti ready.
For a more direct answer, you have for a reason, use it to get in, and spin to get out if needed.
