Its a mixture of everything. The tier list assumes a 1600 - 1700 elo skill level, and ties in the champion's effect on the outcome of the game. For example, AP carries like kennen and casseopeia are designed to do large amounts of damage in early and mid game, and control the gank flow of the game during that time. The better AP carries can do that, generally the better they affect the outcome of the game. There are always exceptions, such as champs that prefer to farm and push over ganking.
Solo tops are somewhat based on ability to farm and sustain, while becoming a force into the late game. Aggressive solo tops that can manage sustain get a plus. Warwick comes to mind as one of the best tops right now.
Ad is one of two things. How well they can dominate in lane and snowball, or how hard they can carry into the late game without losing lane too hard.
Support is based on how well they can... well... support their team, carries in particular. Part of that comes from how effective their lane is, and the other is how effective their mid to late game teamfight is. Janna has a decent lane phase, but has an unparalleled teamfight if played right. Sona has the best lane phase, and her ult is pretty good in teamfights. Taric, on the other hand, is kinda iffy all round. hes just decent. which is why hes lower.
Some champs can perform more than one role. Udyr, Warwick, and Kennen come to mind in particular. The ability to perform multiple roles is huge in draft mode. you can bait a counterpick for solo top, only to find out that the original pick was jungle, not solo top. vice versa also applies.
Champ usage can also have an affect. Anivia comes to mind in particular. She is better than I have her for sure. The problem is, everyone has forgotten how to play her, and the same 1700 skill level that i previously mentioned is lower for her than other champs. The day I see good anivias on a somewhat regular basis, or any anivias at all, i will move her up. I still dont think she is good enough in solo queue.
