It's a question of timing, Swain's ulti's CD isn't that big you can't dodge a bit of a teamfight if they early ignited/grievous wounded you. Just flash out, dish some spells, wait for the debuff to wear off and go back in. Or buy anti-debuff stuff.
I for one think Exhaust frequently damages more Swain; less damage = less healing. Exhaust + Ignite = dead, unless you can, as stated, get some space to breathe and go back in. The good thing is even with Exhaust + Ignite many champs can't solo dispatch him before he's back up to full damage and healing.
--edit-- on the other hand, many other AP carries end up dying without the need of these SS. The difference lies, perhaps, in Swain's attitude and positioning, since he sort of makes a target out of himself.
