Malady because it sucks ;). Then you can have both Atma's Impaler and Guardian Angel for defence, making you very tanky.

Malady does 20 magic damage while applying a 24 magic reduction on your target though. Coupled with the Wit's End and Madred's Bloodrazor, that's a lot of damage. It brings you a lot closer to true damage.
