Mhh, how do you play and do are you serious while playing?
It could be that you are probably good but you lack simple things.
On my way from 0 to 1600 I noticed that pretty often. People who are actually good at playing their lane/role, but lack mapawareness, comprehension of when to engage and when to disengage and panicing. You can avoid wasting many summoner spells or just skills by staying calm and analyzing the situation.
Many players tend to insta flash or stop attacking / run away from someone who relys on skills, thus they just run away from a target that has no way of killing them. Now thats for example.
To analyze your loses I'd need to know more of you, actually ^^

I try to be serious most times, especially with ELO on the line. but there will come a point in the game where I just stop caring, but a that point I am trying to find the answer to winning, but coming up blank.

Impression just from reading your post; you mention 16/14/20 and 20/10/9 as if those are particularly good scores.
Yes you have a lot of kills, but fourteen deaths (or ten) is way too many and completely unnecessary.

all things considering I am playing Katarina for those games. and most of those come out during team fights when I get focused first or second anyway the way I see it, a 3 or even 4 for 1 trade works in the teams favor since katarina isn't a pusher
If you are getting focused first or second as katarina, you are in the wrong place at the wrong time. Katarina isn't a front line champ. She needs to wait until slows/stuns/ults are blown on the tanks and everyone else before she comes in with
Shunpo, smacks everyone with
Bouncing Blades instantly going to sinister steel and cleans up with
Death Lotus, then shes gotta GTFO and wait for her CDs to come off again (if anyone is still left alive). Its all about timing and knowing when its your place to go into a teamfight.

I do wait, as long as I can. but something goes wrong and it jsut turns out to be a bad fight, either the ADC doesn't target the right gu or isn't there to begin with...

Die less, you'll lose less.
That's the only "general" advice i can give.
Most of the other pro tips are lane specific.
Are they attacking the other AD carry?
That's pretty much the only target that can be considered "wrong" for the AD to target.

As Katarina, you might not be a pusher but you have so much escapes that you actually should be really in the worst place possible in a team fight to die this much.
But as any champion, you should always think like this :
If I am destroying the enemy team by myself or with my team, this means that the actual balance of forces is in our favor, while killing them makes us stronger, letting them get a kill can help them build something to turn the game around.
In a Katarina case, you can be, 3/0/X, and then, you die to the ad carry, he uses the shut down gold to build a negatron cloak or finish a banshee's veil, good luck killing him next time.

Always think like this : Winning a team fight > surviving > killing.
What does that mean? Unless you have to die for your team to win a team fight ( If your a tank and are more than happy to soak the damage), then prioritize your survival to killing the enemy.
