You shouldn't put in everything you've seen go mid, because sometimes people play silly things that really aren't standard enough to worry about. This, usually, is because those silly things aren't as good.
It might be worth adding
Kog'Maw, who is still played mid occasionally;
Malzahar, who can't really play any other lane;
Mordekaiser, because he really can't play any other lane;
Pantheon, because Mantheon mid is actually pretty good; and
Swain. There are also some lesser-played APs such as
Tristana and
Xerath that you could add if you're so inclined.
Zed might be a legit addition, as well, but it's probably better to wait a few weeks and see where he stands first.

i think you should write about what you feel comfortable with (have enough info/knowledge about it), but i dont think there is much usefull information regarding specific adc's. indeed, it's out of meta and if you can't deal with it as an ap, there is something wrong with you, i guess.
however, i'm not sure if you should fit gp and pantheon there.

No offence, but I think if you can't beat a Caitlyn or Ashe (particularly Ashe) with Ahri something is going pretty wrong.
AP carries are the natural counter to AD carries, especially when they have burst, hard CC and three gapclosers. That's one of the reasons ADs don't go mid anymore.

The question was not how to counter them..
it was if I should add them to match ups (because I've seen them mid a couple times). You know, like tips and stuff on how to beat them for new players?

It's probably better just to note that ADs rarely go mid and what to do if one does instead of writing match ups for each individual AD.

I know the ones that I have issues with, Kassadin, LeBlanc..
I'm making a master list though, so I can explain midding against as many possibilities as possible.
Now, here is the question..
Should I add in champions that don't fit the meta currently? Like, I used to see Caitlyn and Ashe mid lane and now days I don't. Today, you almost always see an AP in mid, I don't want to seem like an unreliable source if I add in ADs as well.
Should I add everyone I've seen go mid? like Diana and that new Zed guy?
