You can check out my guide, it's a bit out of date but a lot of the information applies.
You want to make sure in every matchup to wait until they burn a big spell to engage on them. For instance -- if you are going against Lux, wait until she tries to bind you; dodge it with your Shunpo and jump on her, then Q and W and possibly ult if you can kill her or get her low enough to force her back. If you just be patient and wait to dodge a big spell, you can engage and win every trade.
With Twisted Fate, wait until he uses a red or blue card to farm; then quickly spam your abilities on him and back out. That's how you win trades with Kat.
If you are in a hard lane where you can't win trade, or the enemy has high sustain to keep up with your harass, such as Lee Sin, you might have difficulty farming -- if that is so, just wait until the creeps hit tower and then Q and W them when they are low. You should be able to wave clearly very effectively this way. Clear the wraith camp if your jungler isn't by if your lane is shoved and you can't cs.

Akali is borderline OP right now, she is pretty hard to play against. Xerath is ok vs her though, as well as other people with high range -- just poke her and try to zone her pre-6 as hard you can, and if she engages stun her so she can't kill you (since you play both xerath and brand you can stun her with both).
Zed is also pretty hard, get an early seeker's armguard, sit out of range of his melee so he can't spam E you and try as hard as you can to dodge his shurekins. Post 6 he can full to nothing you though, so unless you poke him down well you should never try to engage him without a gank, just do your best to farm and pick up wraiths.

Arcana's points are legitimate.
However, in Wrath's defense (Devil's advocate?) there are two semi-independent systems running hand in hand now. You still have an Elo system in your matchmaking, and thusly, your Elo can fluctuate faster than your divisions can keep up sometimes. It's possible that Wrath is being matched with high plat / low diamond players because his Elo has raised up faster than his divisions.
Transversely, if he loses a lot of games and stays in his divisions, he could be matched against gold/platinum players and be platinum.
The League system is not a very good measurement of skill for this reason. Then again, neither was Elo either.

Yea it's weird. But since my rating is so high, I automatically skipped Plat IV and went to III. I have yet to play a game in III yet but there's a good chance I'll skip from III to I as well.
