I really don't know how to respond to all of these comments, but they bug me.
1. I shouldn't have to hold your hand to show you a fun way to play a champ.
2. Go try it, practice makes perfect, not reading about every possible line up and match a Teemo has ever had.
3. Not 2 cutlass's, a BOTRK and a HEXGUN, they don't have redundant factors there. They give lots of AP, AD, AS, slow, lifesteal, vamp, etc...
4. If you want each item explained, read their explanation on LOL's website
5. Don't hate me (or my guide) for my opinions :P

If you have a real concern, try to tell me and I will solve it :)

There is a loooooot that can improve.

Let's start with the basics:
1. Please add summoner spells.
2.
This is not an item section. In an item section you explain each item individually.
Each item you wrote down in your cheatsheet aka your build.
3. Put the spells chapter before the item section, not after it.
4. "In closing" should be "In conclusion"

Fix these things and your guide will already improve a lot.
Still some other minor things that can improve, but that comes later :P

PS:
In your rune section you advice people to buy champions instead of runes. I must disagree heavily there. For lvl 15- you might want to test certain champions and buy 1 or 2 champions that fit you. After that, you start sparing IP for your runes, this is very important. Because when you are lvl 20, you can buy all your tier 3 runes instantly, which is very useful indeed.

Just some constructive criticism


Buying two cutlass based items is a bit redundant. I suggest trying Guinsoo's Rageblade in place of Hextech Gunblade on hybrid. You said buying both of these items allows you to stay on top of a running champion, but that is why you should almost always buy a Frozen Mallet on Teemo. Since his hard engage is rather weak, he relies on a lot of kiting, and the 700 health will greatly increase his lifespan.

You take scaling magic resist Glyphs suggesting you are laning against an AD based champion, but take Resistance instead of Hardiness , making your masteries and runes a bit inconsistent.

I am not going to upvote or downvote this guide as it is clearly incomplete. It needs a lot more work including matchups and expanding on strategies for top teemo...shroom placement guides...etc.

The item build and masteries for the hybrid top are pretty good imo (I would replace Hextech Gunblade with Frozen Mallet. The item build and masteries for AP is pretty good too (I would flip the order...flip Liandry's Torment with Nashor's Tooth) though the runes leave you awfully squishy for a top AP teemo...you will REALLY have to be cautious with nothing but scaling AP runes.

Keep working on it (I like the approach you are taking as a guide for top teemo as that is really the best way to run him) and I will come back when you get a little more information in it.
