Well just a tip, I went on an extremely large tilt after hitting plat, and even after taking a week's break from ranked, I still continued on the same tilt. I was about to give up, but I just kept on playing and eventually I managed to fix my MMR and hit plat IV too.

As impossible as it may seem to climb now, just keep playing and you'll eventually climb. You can only get unlucky for so long.

I'm not sure what I'm supposed to do in games like these. I dominate my lane. I roam and gank sidelanes. I call mias, tell them where hes going and follow. I farm almost perfectly. I push his turret super fast. I focus their damage in teamfights. I only die once, during a teamfight. I make plays and catch out enemies with my ult combo. I'm not sure what else I can be doing.

I'll tell you what to do after games like that: you should be happy.

Yes, it is frustrating to lose games in which you feel you've done well, in games in which you feel you didn't deserve losing, but let's be honest: that's how this game works. In the end, the only way you influence a game is by your performance (most important) and your attitude/behaviour (also not to be underestimated).

Judging by the screenshot, you did most of the things in game right and you most definitely played above a Gold V level. Of course there's still always room for improvement; whether it's something you did wrong or something you should have done (but didn't) does not matter, always be critical when it comes to judging your own performance. Personally I also wouldn't pick Malzahar because I don't like him, but let's ignore that for a second.

Judging by what I've seen from your attitude in this thread, the behaviour aspect might need work, but as long as you're not actively flaming others in game, that's a good start. It's incredibly easy to go on tilt during a losing spree and that is the one thing you need to actively avoid at all times. If your team gives First Blood and you find yourself thinking "well, gg, another bot lane feeding and losing us the game", that is a clear sign for you to stop playing. Mentality is huge, remember that.

Anyway, if both performance and attitude are at the right level, don't worry about losses so much and be happy about having played well. If you play consistently well, chances of you being held down only because of bad luck are incredibly slim (which is also why Lord Kesharq's comment made me lol: there's a reason you're stuck in Bronze V, buddy). If you still feel luck is too big of a factor, get someone to regularly duo queue with. Stop worrying about teammates, stop even mentioning them in after game chat or in this thread. Focus on yourself, it's the only way to improve and - as a result of that - climb.

And luck is a big factor and its not on my side... I've had literally 0 enemy afks in over 150 Gold matches and at least 20 allied afks... here's an example... also everybody else on the team was horrible
We could have won 4v5 if Xin and Wukong knew what they were doing

And when I see a fb on any lane I think to myself that they may have just made a mistake but the second I see a second death on them for no reason I start to think to myself that they cant play... I don't start flaming and still play my 100%, maybe even my 110% since I know that I have to make up for the weak link
