Yes I believe I do, most of my deaths I think come from over extended, lack of wards, and staying to long after a team fights and silly things like that. (I think)
That's a good place to start. If you have a friend, get them to remind you to watch the map every 3s. Other than that, you need to have good intuition for when the map is just too dark.

I don't play ranked yet, but I notice you die quite a bit. This has been what I have been working on to prepare for ranked, and it has helped me tremendously. I also notice you've done pretty well with a range of champions (
Darius,
Akali,
Annie), but your CS is quite low on each of them. I might recommend zoning in on what role you like to play, and just really get that down.
Of course, sticking to developing mechanics will help you out the most, so last-hitting and not dying should be at the top of your list. I think it's something like 12-15 CS ~ 1 kill. Many pros have 100 CS by 15, or even 12 minutes (~8 kills). So that's something to think about.

One of the best ways to get better is to stop playing ranked, it puts a lot of pressure on you, which can make it harder to learn, some people learn better under stress. While others get better by working on things in nomals blind/drafts, things like
*map awareness
*CS
*KDA ratios
*Mechanics
*Securing objectives
*Zoning

Pick a role.
Pick a champion for that role.
Pick another champion for that role.
Stick to those champions/roles as much as you can.
You play too many champions to be effective with each.
Every champion takes time to learn. To learn how the champion fares against many different opponents. By playing a wide variety of champions, you never really learn how to use them.
Do not try to be a hero.
If an ally is in danger do not just jump in to help him. Assess the situation. How many enemies are present. How many are close. Check mana (if/when mana used). Check health. Do you have your abilities ready. What routes are available to escape should things turn bad. Is there a realistic chance you can save your ally without dying. Can you kill the enemy.
If it's 2 vs 3 and I'm not convinced I can take them, I run. Should my ally die then it's a pity, but it's better to let one die than trying to save him and both of you dying.
You will get grief over this in your current elo, but sometimes helping an ally isn't helping the team.
Last hitting.
Check some streams of high elo players, look how they do it and learn from it. It has helped me so much.
Managing your lane.
Learn how to retreat without losing cs. Learn how to freeze your lane.
Most people in your elo think it's best to just push as hard/fast as you can. And keep the enemy under their tower.
This is a big mistake. It opens you up to ganks. It makes it very hard for your jungler to gank for you.
Objectives win the game.
Don't go after kills. Like really. I've seen my share of games lost because people go for the kill in stead of taking the tower. If you see an opportunity to take an objective, be it a tower or dragon, and you are sure you can take it, go for it.
It's not all that rare for me to have 3+ tower destructions on my name. That's how I try to win games.
Be aware of your enemy.
If your enemy laner goes missing, tell your team. Use this time to either push his tower (denying him farm) or roam yourself. DO NOT just sit at your tower waiting for minions. Sitting at your tower waiting is wasted time. Check the status of your team and help getting an objective. Be it the tower in your lane or helping your jungler in a counter jungle move or help one of your lanes.
Learn timings.
Blue and red buff spawn at 1:55 and then 5 mins after they were killed. This means at 7:00 to 7:30 ish, your enemy will take blue buff. Either the jungler alone or trying to give it to mid.
Depending on your role/lane, this is a time you can use to make plays without immediate fear of getting ganked, or time to be carefull because mid and jungler are close to you.
The minimap, your mega friend.
It's there. It's invaluable. Use it. Don't get too focussed on your lane. Check the minimap often. Before engaging or trying to make plays, check the map and hit tab to see who's alive.
Wards.
They are your friends. Do not leave the store without getting at least one.
What's the enemy wearing.
Use tab to keep track of what the enemy has bought. Be it items or wards, once he gets back make a point of checking his inventory.
General reminders.
Do not facecheck bushes.
Do not get baited. Learn to recognize a bait and do not get caught.
Objective > kills. If your enemy has little health and is running. Do not follow him intent on getting the kill, get the tower in stead.
When you get ganked. And you are low. And the enemy is low. Do not attempt to go for the kill. On the champions I play I will kill you when you do it. It's a free kill for me and a lost tower for you. Get out, heal up and get back asap.
When low, retreat far enough. I can't tell you how many times I've killed/been killed because I/the enemy didn't retreat far enough before backing.
