I came up with a champion idea off a mounted warrior, striking hard with a charge and shooting arrows at his enemies while drawing back.
as my first concepts for champions tend to suck, ill keep this version a bit raw in the beginning.

I want to force the player to play both sides of this champ, so im thinking of giving him high cooldowns independent of the other side of the ability, to force him to switch depending on which side is on cooldown, or making it so that the abilities do not replenish without switching weapons/ using the counterpart.

Finally a concept I like at first sight^^. Still got a few queries, though:

Usually champs with multiple ability kits use the switch ability as their ultimate, as it would be sort of OP to have 2 ultimates ready. Not sure about the ults tho as his melee ultimate does kind of suck for engagements and would be broken for escaping (Thought he was supposed to enter the fight melee, and leave it ranged, correct me if I'm wrong). You should probably just switch the ultimates around, having the retreat ult for bow form and jumping into the fray with his melee ultimate (which would be the fly+fear thingy, sort of like Onslaught of Shadows). And switch your Q to R to turn the six abilities into normal ones (long cooldown around 30 seconds is totally ok for the ult-like abilities.)

Heavy Swing: Knockback needs to be dependant on hp, not size. Or it is too random with small tanks like Rammus or big carries like... hm, well, rammus is enough to break the size concept.

Whistling Arrow: Shorten flight time. Not only will this make the feartime less broken (you could shorten the cooldown for that to make it a normal ability), it will also stop champions from walking faster than your arrow flies (which they can do if you only travel with a speed of 666 (evil, btw.)) which would just look odd :/. Probably not that odd if you make it to a melee engagement tool, though.

Movespeed thingy: If you want to flee with this in ranged form, you should be able to kite and attack enemies. That won't work if the bonus is gone when you attack. Better give some flat MS and tenacity bonus and add bonus MS if you hit enemies with AA, which encourages kiting instead of just running and doing nothing.

Just some rough thoughts, correct me if I understood some part of the champ wrong.

you should have noticed by now, that i don't like to follow the normal ways, so i take my ideas here and wait for people like you to knock some sense of reallity into my head^^

@heavy swing: i didn't know there was a special measurement for size/weight of champions in this game, so i eventually would have made it dependant of the max HP anyways...
