The combo is: (HF) q + w + e + r(SF) q + w (+e)* if the enemy try excape.
imformation:
In SF the habilities dont spend mana, use it for resist in your lane if you havent mana.
If you have few health use w in SF and atack minions for get health.
SF is a good form to farm speedly
Stay everytime in HF Ç(when you can) because her e is a good skill for excape, and when you go to atack enemy (very important) use first HF secuency and later SF secuency.
When you change your form, the cooldown on your habilities changes to the others habilities, for example: If you are in HF and use q, and later use r for SF, your q is prepared to use, is another hability But if you turn on HF the cooldown of your q(HF) dont be prepared.
