Well. The answer is simple if you look at the items Unique Passive its : +1 Attack Damage for every 2.5% health missing. So when you use your ulti with 100-200 hp you can make pentakill easyly. At the other side it has "Unique Passive: If you would take magic damage which would leave you at less than 30% of your maximum health, you gain a shield which absorbs 400 magic damage for 5 seconds." so its harder to die.

The other items are normal and for pushing and getting kills faster.

At lane try to get first blood and rush for berserkers. After that dont leave the lane untill you have enough gold to purchase Vampric Stealer. When you return to your lane farm minions and buy BF Sword. After that rush to your Blood Thrister and destroy the jungle near your lane. And try to get kills for your BT's stack.

In teamfights never scare to dive (only if you dont have ulti) Your maw of malmortius will give you at least 2 kills :). After teamfight dont forget to run with your "Spinning Slash"
