Well, I keep it unlocked, since it gives you a much better sight overall. However, teamfights can go a bit messy, so locking on your champion during teamfight can be a good idea.
The rotating thing is only possible with LOLreplay
Not sure if gfx will improve your clicking. Never thought of that, honestly o.o

Switching between the two is the better option IMO. When you're heading to a lane or something like that then not locking your camera is fin. Locking it on the move from bot or top to mid or vice versa is better. Moving around in the jungle lock it....It's really on a situational basis.
unlocked, much more flexible. Moving to specific areas of the map on the minimap is pretty difficult sometimes, even if LoL is kind enough to show you a pathing line. Also its hard to distinguish whats going on in a fight for the use of global ults like those of Soraka, Gankplank, or Shen if all you can see is around you. I've never met someone with a high rating that locks their camera for a Moba game tbh.
not sure on the other two, hope this helps though

How do you make the camera zoom rotate vertically? (As in I go from looking at the champions from above to looking at the champions from the side)
Do you think having gfx turned up helps with accurate clicking?
