Can't you like... bind the mouse keys to keyboard letters out of LoL, and then use all 6 buttons like that?
p.s. f u guys and your fancy mice. I carried my normals with a trackpad until I sold my laptop to build a computer. xP (of course, this is why I played junglers and supports. Not as much last hitting LOL).

There is a TON of games that only handles 3-5 mouse buttons and the mouse-wheel is included in that so any more than 2 thumb buttons is rarely even usable and this mouse has 6...
The buttons also seem to be a little too far back to me. They seem like they might get in the way for comfort.
Most times muliable button like my razer naga at home has twelve buttons on it and you bind those keys to whatever you want lime button number five can be bound to e if you wanted :)

6 thumb buttons is too much in any case, because you'll still be doing the aiming and clicking with the mouse so giving the same hand more things to do would rather slow things down than speed them up. Right now the tasks of control are pretty divided between the hands.
I think 2 is the perfect amount of thumb buttons. They won't get in the way and are easily accessible.
Because of the layout, i only use 1 and 4.
The thumb buttons are really out of the way on the naga because the mouse is so xbox hueg.

That's not as bad as I thought it would be. Still not comparable to the ~40 you can get a good gaming mouse for. Any more than 50 is a bit of a waste imo.
