"The reason it even remotely works is because Garen is AD and mages have very, very little armor. The silence helps too.
If they are a strong kiter, Garen go down fairly easily since he has no reliable gap closer."

"I'm going to have to go with the anecdotal evidence of Xin and Nasus.
However, Nasus was a bug where he had a 1:1 ratio on the HP drain for his ult and he could 1shot anyone once he got 100 AP. That was fixed fairly quickly though.

"If they're going to hit him, it'd probably be the armor shred on the active portion of his shatter or the passive armor to others to promote aggressive play.
Most obvious though is changing the heal amount if used on someone else instead of yourself."
