I only read the first few paragraphs since I don't want to embed knowledge that's outdated, but you seem like you knew what you were talking about. Rebuild it from the ground up, please. :)
It'd be great if you consider my points when developing the guide further more, so people are not scared away from this extremely useful champion;
From what I've played in my current tier (Diamond III), I can safely say that Ivern is an extremely strong champion in terms of utility in late game, and combined with both Rylai's Crystal Scepter & Locket of the Iron Solari, he can give massive shields later in the game. I was capable of nullifying most of a Veigar's min-45 damage from my ad carry which had a pure-damage build (reaching about 1110 AP)
Having a good communication with your team helps you a lot early in the game, as people will tend to counter-jungle by stealing your buffs because they suppose that you already did theirs, which ends almost always in an early first blood in favor of your team if your support wards properly.
His "Brushmaker" is also extremely useful when counter-ganking, so I would say that Ivern focuses mostly in map control in a defensive form (being Rek'Sai an example of an aggressive one) early in the game (counterganking, jungle control, ward control, etc) and in my opinion it is very important to get "Sweeping Lens" and "Boots of Mobility" as soon as you can (not before "Stalker's Blade" though). Being aware of the enemy jungler is essential and will help your team tremendously.

For the next item i see obs jungle item get tank or ap depending on team comp and damage, and either trakers knife for vision or stalkers blade for catchup.
Boots always ionian boots of lucidity although i see why some might do better than others, again choose this based on team.

Anyways, I had to upvote your guide because I was struggling with this guys, failing every game. I was sure I was playing him poorly. I was building the right items, just not in the right order.
My problem wasn't the build, but the gameplay. I kept trying to be a damage-dealing champion. I hadn't realized he's focused on a supporting role. First game after reading this: Got an S+.

P.S.2: The mastery section, where it says "It also pairs well with the Tier 4 mastery Perseverance" is linking Garen's passive and not the mastery.

I totally get where you're coming from. There are a few things that I've come across in my time with Ivern that seem to help out in these situations a ton:
a) Max E first. We almost always want to max Q first because of the root duration increase (and CDR/damage adjustments), BUT E isn't a bad first max at all. It's excellent for continually peeling during team fights, and having it at its maximum potency as early as possible can be a huge asset. It's best used on your divers (initiators/juggernauts/sticky tanks) but can be just as useful for a carry that's being jumped on early in fights. Learn from each team fight and decide who the first shield of a fight would be best used on.
b) Take Rabadon's instead of Rylai's. The more I try this in my games, the more I question Rylai's necessity in the build. I think Rylai's would synergize much better if we had Nashor's Tooth (which we're not going to build in our regular path). Rabadon's is going to make Daisy HUGE! All of her damage/stats/resistances are buffed based on your AP, and Rabadon's is THE ultimate source of AP. You'll see a huge jump in effectiveness from Daisy after you complete this item, and it won't be such a bad thing that she's one of your only methods of peeling.
c) Take an early second point in W and save it for team fights. Creating brush has many uses. Creating siege scenarios at turrets, giving your lanes huge pressure, etc. - but one of the major uses that people are forgetting about is that it's great for saving your teammates. When a team fight erupts, drop a brush down on your back line carries. Even if their team KNOWS that they're in the brush, they won't be immediately able to pounce on them, giving them a few more moments of DPS and kiting. You can also throw down a few other scattered brush to confuse the enemy as to which brush your carries are actually in. Brushmaker has amazing potential - get creative with it! :)

I agree, honestly. I have some (rather) large changes coming to his item path. I'm hoping to get them out either today or tomorrow. I no longer feel Rylai's is core on him - especially when he's not building Nashor's. You have to drop a brush on yourself to even properly benefit from Rylai's, but more often than not, that brush should be used in more clever ways during fights to peel your carries.
I've also been messing around with Ardent Censer for a few days now, and am absolutely LOVING the results. It might become core to his supportive build (I have more reasoning for this in a response lower in the comment chain to Lars Lyon).
