As far as content wise, I cannot say much since I don't play Karthus ADC but what do you think about Luden's Tempest? I just saw some Karthus ADC players take that to help with mana. Speaking of mana, have you ever considered skipping Tear of the Goddess entirely? Presence of Mind restores massive amount of mana so I found tear to be overkill in my own experience. Tear is a scaling item by nature, so skipping it allows me to get more power, sooner. Just a thought.
Also if you could work on a match-ups section, this guide would be even better! I know it takes time though.

Might just get me to play Karthus honestly since he does seem like a really fun champion.
I wonder tho, have you ever tried any other rune? or is Dark Harvest the only one you have used? I've heard Predator is supposed to be pretty good on Karthus aswell but yeah.
You should really give Karthus a try. Getting kills with Requiem is just so satisfying.
As for bringing Predator instead of Dark Harvest, Predator offers better damage compared to Dark Harvest at 0 stacks, but it's so easy to get stacks because you literally just have to press R when there are enemies below 50%. I've heard going sorcery second for manaflow band can help your laning phase, but if you're smart about how you use your Lay Waste, presence of mind's CD will be more useful later.
