There's a ton of "open-air" fighting that needs to be done against her. Her charge stuns you near walls so, jungle fighting's out. You can't really have your whole team focus her because she'll just ult the weakest of you and run from a 5v1... I think she's a little OP really. I don't see her much though so, it's all good for me.
To be honest, any champion is that good when fed. If a Poppy is hitting that much, they probably won't have over 2200-2500 hp, and if your team focuses them, they're gone.
Really though, the best solution is to never let them get kills or gold from minions in the first place.
As for Nasus, he can be tough. I recommend you play a game or two as him and Poppy though. That's the best way to find their weaknesses. I am terrible with Poppy, and decent with Nasus, but what I found realy hurts as Nasus is Ignite. It takes away 50% of my healing, including lifesteal, which is a major part of Nasus' staying alive. Also, make sure you have a DPS on your team or you'll never kill him, a good Yi, Akali, Katarina, anyone with very high damage potential and a
Madred's Bloodrazor is your best bet.
Remember that tanks still get 70% money from assists, so zone them away from their team if you can.
EDIT: You want least 2 forms of stun on your team, preferably 4 or 5. With stun or root etc, Poppy will have tons of trouble making it away from your team in time. Nasus is less vulnerable to this, but stuns always help.

@Lugignaf: It is kind've irreleveant of "Open air fighting" If she can catch up to you, and even using that stun doesn't mean nothing she will find a way to catch you.
@Lazukin: We had two roots, two stun, one fling, and two slows. (Their comp was about the same. Though when she pops her ult its just about a guarnteed kill. Ex: tried to save Irelia from Poppy , who used her ult, Gold card away! Ignored. I ended up killing her, but most attempts to stun often are worthless even if its the targeted person. Maxed out pick a card lasts two seconds, she still caught up to me.
My proposed solution: Change her ult a bit and make her vunerable to stuns, slows, etc. For a shorter duration. This shouldn't be too drastic as all she has to do is pack, cleanse and a BV.
As for Nasus I probaly should have done that, but I forgot. Ah well live and learn gotta remeber my situational items.

Again though, was she fed at all? A Poppy isn't just gonna pop out and 2 hit you unless she's fed or you have like 1500 hp and no armor. I can't think of one champion that's not 95% unstoppable when severely fed. Once you do kill them, they're still 90% unstoppable :S
Yeah she was, and that kind makes this topic irrelevant. But Lux and I had our fair share of kills. I kept her shutdown throughout the early game though, but after that she must have gone to do something else. That and she would usually pick the biggest threat with her ult just about balancing how many kills we had, and proceded to kill us. Like I mentioned before there is no stopping her once she targets you, your tank can no longer save you, its up to you.
