You just have to keep on playing man. Pick a champion to 'main' and stick with it for a while. Know the role of that champion and what you have to do for the team. Practice getting last hits and DO NOT fall into the abysmal trap of blaming others like you're some l33tsauce. People in these DOTa-like games tend to be full of themselves and lack humility. If someone trolls you don't say anything and just say 'sorry man,' if they blame you for something, u can always play another round.
Try to play frequently with a group of people you meet in the game or you know from other places. Get the vent/skype info and communicate.
Main thing is to have fun :)

Everyone can always improve, just think after every teamfight, etc, what could I have done better then?
Ofcourse, there's is sometimes ones where you don't do anything wrong, but that's the nature of a team game.

@SatansFate That is pretty much what I am currently doing. Beating myself up over mistakes I make and what I could have done better.
@Jebus I do that often, I generally watch jiji streaming. I find it's been pretty helpful. Haven't really seen anything on TreeEskimo though.

The best way to get better: just pay attention.
Seriously, look into all the champions and make sure you have a pretty good idea of them and what they do. This way, you know how to fight against, alongside, and with them in a game. You can also take everything you have done in the past and analyze it. There is always something you could have done better. If you die in a game, you messed up. If somebody got away from your onslaught, you messed up. It's that simple. It doesn't mean you completely screwed up, but it wasn't perfect (nothing is). You just need to pay attention to where people are, what they can do, how to handle enemies/allies, and where/how you are positioning yourself.
Going in a pre-made game with a full team of your friends on Vent/Skype can solve a lot of problems, but not nearly everything.
