Another item that might be noteworthy against AA's, thornmail, giving a chunky 100 armour, as well as the AA return damage.
I generally test stuff more in 3v3, due to the shorter games, although it can have a negative impact in being too short, so really its just starting builds, which I think I can come to a close on for my personal opinion, so I'll prolly give 5v5 a go to try out some full builds later.
It sucks people vote guides down because they also have a Vayne guide or just because its not the same as every other build/their build.
It's not so much that you PLAY defensively with this build as you focus on having good defensive stats for late game so you can continue to be offensive with your bolts.
I've honestly never played 3v3 and never intend to. I came to LoL after playing DotA so 5v5 is just sort of what I do haha, but I would imagine the games would be a lot shorter in comparison to 5v5. I like your alternative item choices though, aside from Manamune. I can't recall ever having experienced mana problems after the very early laning phases of the game, but if you do have noticeable mana shortages then by all means go for it. Hextech, like you said, would really be the last thing you get because of the wasted stats. The only reason I would probably even consider getting it at all is because of the active. If it was just the AD/AP and lifesteal/spell vamp then I wouldn't even bother me thinks.
It's really a shame that people downrate the build without reading it because of a lack of AD. I guess I should try to make a note of the percentage damage in the title..
I suppose it also varies on who your against, if they are building resist because your team members are AP heavy, then black cleaver is best, but if they build armour, because your the carry, then madreds is better.
I also find trinity isn't... viable, due to its great cost, which usually leaves me building sheen, and the game ending before I can even get trinity, I may try out some replacements for it, although sheen/trinity allow for that extra 3rd hit nuking potential.
I've been playing Vayne much more offensively using bolts, but I will give it a try your way and see how going more defensive gets me.

Build is focused on % over AD, if you want AD go look at any other Vayne guide, it will teach you how to kill squishier squishies, as opposed to this killing nearly everything at the same speed.
I did do some defensive testing to find,
3v3, its pointless going defensive, once you have madreds, if the game isn't over already, by the time you finish building your speed boost, it will probably be over.
I'd even go as far as saying you may as well play a 'cheap item' build for 3v3.
BoS + 3x HP
7535 is the total cost of the above, and out of 5, 3v3 games earlier, I'd only made more than that once.Zerk greaves

5v5, I'm still testing, a few items that are noteworthy below.
Malady (50% atk spd, 20 m.dmg, m.pene [4 stacks*6]) not quite as good as either phantom or wits, but a little bit of both for a LOT less
Manamune (20 AD, 1350 mana, 7 per 5 mana, 2% mana turned to AD) cures mana problems from team fights, as well as adding 47 AD by itself, not including bade/any other mana
Hextech Gunblade (60 AD, 20 life steal, 25 spell vamp, [active]) this is the only viable life steal option, due to low AD because of the aim on bolts the life steal isn't up to much, but the spell vamp will work with Madreds/Wits however, however it is a very end-game option, due to its price and lack of necessity
As for the Black Cleaver, I opted not to go with it in favor of items like Wit's End. I wanted a build that had good defense while still allowing you to have great output in terms of damage. Replacing something with Black Cleaver would work against that principle. Even had it been included, I would get Madred's before it because it synergizes well with Silver Bolts, which is what I max first.
Sheen > Trinity Force is a viable alternative in place of Wit's End, but I just prefer Wit's End. I'll make note of it though.
I do however strongly agree with you on Silver Bolts. Its damage potential seems to be highly underrated thus far. At level 5 it deals 8% of the target's max health + 70 damage as true damage. It really makes you hit like a truck. If you do it right, you can harrass like none other in the laning phase.
Tumble is your bread and butter spell. At rank 5, it's at a 2 second cooldown. Why in God's name wouldn't you max is first?
I have tried multiple builds with Vayne so far, and I have to say getting bolts first has seemed to work much better.
Changes I'd make/consider to make to your guide;
Starting purchase, 'Boots of Speed' + 3x 'Health Potion'
Reasons for the above, boots + potions should keep you out long enough to get a B.F. Sword, if you skip potions then you should at least be good to get a dagger if you need to base early, black cleaver is used because it gives attack speed, damage, and the passive 45 pene works nice with the 3-hit of the bolts.Rush build 'The Black Cleaver'
Build a 'Sheen'
Build a 'Madred's Razors'
Upgrade shoes, Merc if they have a lot of CC, Zerk if they don't
Sheen is next for the proc from tumble to go with the 3rd hit of bolts, allowing you to easily take off 50%+ of an enemies health. Follow this with razors (not bloodrazor) for the 500 bonus, as this will easily allow you to take down the dragon on your own as soon as you get it, which can be achieved before level 10 fairly easily. This then follows onto your guide more, by upgrading shoes, getting bloodrazor, and then whatever else depending on the enemy teams champions.

This build's main focus is on Silver Bolts. If one wanted to, they could max out Tumble first, but I prefer having Silver Bolts maxed over it.
Edit: But either way, I find that one level of Tumble is more than enough for use during low levels.
