i didnt use the tiger form (only at the end of the game i used it).
The Items, Abilities and Runes are synergizing great. It doesn't have jungle items like most other Lanedyr builds while quickly emptying the jungle for money/exp is still easy. I think this build shows good why Udyr is not classified as jungler.
Well I think that Udyr (especially without runes ಠ_ಠ) is so much better in lanes anyway... besides the fact that Lanedyr is more fun to play and is so much more horrifying for the enemy team.
BUT I think it is missing out some mix ups to adjust to friends and foes. :D
Here some mix ups I like to use:
Abilities: I 100% agree with the end result (5/5/3/5) but especially with Udyr you want to lvl up what is needed most. For example: If you're in a sidelane with a char that can easily kill an enemy at the beginning - bear first -> Stun -> Your team gets first kill.
If you could take down towers quickly or need point dmg -> Tiger...blablabla... Well I think every Udyr player knows that you have to mix the abilities a bit up ;)
Items: A recommend the original in this guide BUT you can mix up this a bit to get not better but slightly different results.
I like to buy the heart of gold before sheen - over all i seem to get my triforce a bit earlier but I would only recommend it if you're patient (since you need sheen a lot)
If the enemy team don't have a stun character a heavely recommend to use the Ionian Boots instead of the Mercury's Treads. Cooldown reduction! Useful!
What was already said: Mix up which armor you want first ;) If there are not many mages in the enemy team (or in your lane if that still is going on when you get to that point) Pick up the Spirit Visage first.
Lastly: If you don't feel like that you need Altma's Impaler or think that you don't have the money (and don't plan to ever come back) you could use the Philosophers stone and Upgrade it to Shurelya's Reverie. You cooldown is ridiculous low and your dancing just like crazy (including insane amounts of turtle health)
I also experimented with using Youmuu's Ghostblade (Tiger + unique active: 50% attack speed for 4 seconds) but I'm still not sure when to buy (I'm not even sure if it is THAT good) it and what to drop for it (most likely Altma... but Altma is the better Item... well it depends on what you need I guess :/ )
This are just SOME mix ups ad I still recommend using the original build if you're unsure.
There are situational times when spirit visage is the wrong choice, also. Sometimes the team is VERY cc heavy, so grabbing a BV to rid of an extra CC spell can be helpful. Or, they are just over ran with AD, and getting a Sunfire Cape works well (armor+hp, and that HP converts to AD).

With 1000+ wins under my name, i can assure you that this build works, and works well.
Sometimes I still find myself in a lane, but for the consistency that jungling offers, I'll gladly take Smite over a more useful late-game spell. Laning isn't something I plan on or build around, and for me, the result has been the near elimination of bad games. Mediocre sometimes, but rarely bad.
Not putting in a negative vote because I haven't tried this build, just stating my experience with lane Udyr.
