I just use shift and alt for smart/self casting reasons. It's easier to adapt to the situation (e.g. if I'm aiming a click-shot or an AoE, I want different casting options).
Although, self-casting is still hard. Alt + Q/W/E is a weird position for my hand sometimes.

I also sometimes click on a skill to see the range so binding smartcast somtime screws me up. So many times when I accidentally use a flag toss on Jarvan when I didn't mean to.
