Not sure about the passive. If you were already in melee range for 3 attacks, why would you need range for the fourth? Maybe you could change it so that after a couple of seconds your next attack will be ranged.
Q is sort of weak right now, I would add another component to it (maybe damage or something of the sort)
W can have higher base damage, I like the spinning though XD
E passive I don't think is good. We already have Hiten Style, the heal itself is weak, and the heal doesn't really fit his kit. The active could use higher base damage, but otherwise is fine.
I would reduce the base damage of the ultimate at higher levels, otherwise it's fine.

Assuming your stats are using the Malzahar model, using HP per level yellows, you'll have 4500 HP with those items! I am assuming that your plan would be to have higher base attack damage than Malz, but, even with just the base AD than malz has, using these bad item, you would have 203 AD.
4495 HP,
203 AD,
444 AP,
armor MR yada yada.
80% of 203 = 162 DMG
80% of 444 = 355
total of 517 DMG. TWICE, LOL.
So your ult would deal (Using this bad tanky build with 4500 HP) 817 + 867 DMG. CAN YOU SAY LAWL? 1684 DMG in one skill is the kind of damage you should need to have 800 AP to achieve.
Malzahar with 800 AP and a full channeled ult only deals 1590 DMG and he wouldn't have 4500 HP if he reached 800 AP, trust me. He'd also have to have a fully stacked Mejais.
So yeah, ult and mana regen per hit opie~

You may need to nerf the base value as well;

That's still too high for a champion who can have 4500 HP :P
And the mana burn per attack is still really high. Having mana burn scale off of AD isn't going to work :P

Story : They say that there is good even in the worst places, that’s the case with a young boy from Zaun ,Tsil. Even though he was born in such a terrible city he didn’t give in to it’s evil. Everything was going as usual the day his parents dies. He came back from the outside when he saw his parents getting taken away. They were hanged by the crime they have not done.
From that day Tsil trained to take revenge he went even to Ionia isle to learn from Valorn’s best masters. But he never truly found his style. Until one day while he was travelling from Bandle city to demacia through Shurima desert, he noticed a tomb. In that tomb he found the perfect weapon that he was searching for, an enchanted rope, when he took the rope all his anger and lust for revenge turned to magical power. He didn’t want to take revenge anymore so without a further goal, he went to the institute of war.

Role : Hybrid carry, maybe a jungler
Abilities : 1. (Unbalanced power(passive)) After 3 seconds Tsil's next attack range is increased by 500
2. (Break weapon) Tsil uses his rope to tangle the target’s weapon decreasing his attack damage by 10/20/30/40/50 (+10% of his ability power and + 5 % of his attack damage) trasnfering the AD to himself for 5 seconds
3. (Spin slash) Tsil spins his rope around the targeted champion dealing 50/100/150/200/250 (+40% of his ability power and + 50 % of his attack damage) then he pulls the rope back making the target spin in random direction
4. (Magic surge) (Passive) Tsil each attack steals some of the target’s magical power restoring Tsil's mana and burning the target's for 5/10/15/20/25 (+ 10 % of his attack damage) each hit
(Active) Tsil shoots magical power at the targeted enemy dealing 40/80/120/160/200 (+ 60 % of his ability power + 10 % of his attack damage)
5. (Strangle) (Ultimate) Tsil throws both ends of his rope to targeted champion pulling him and dealing 100/200/300 (+ 60 % of his ability power and + 60 % of his attack damage) after that he channels for 2 seconds dealing additional 150/250/350 (+ 70 % of his ability power and + 70 % of his attack damage) after the channel the target is stuned.
