This SHOULD be a normal guide.
As a comment, I suggest you to leave the "D mode" (I mean, replace the runes of armor pen, armor, and magic resist). You actually have tons of armor with just the skills, and you can reach some magic resist. if you need by the items.
The best way (at least my way) to start is with full critical chance. This means your masteries, but changing the extra near-no-health for +4% critical chance. In that way, you can start with nearly 20% or 22% of critical.
Remember: You regenerate extra mana for a % of your hit´s damage. If you hit a lot, you regen a lot.
In the very first stage of the game, if you can solo and just hug the tower, you can survive and gain lvl quickly, also letting another team mate to grow quickly too by junlging.
Choosing "Imbue" as first skill you can last till lvl 4 without problems, having a pair of critical chance gloves. Then, Avarice Blade, and then your way to Manamune.
Diferrent to you, I prefer more damage, movement speed, and critical chance items. As an example, Berserker´s Graves, Bloodthirster, Phantom Dancer, Black Cleaver,... But before that, and after the Manamune, I think its necesary a Philo. Stone, cause the extra gold, the mana, and the regen.
At last, if you can reach all that, you could end with nearly 300 damage, an impressive movement speed, 1.5 of attack speed, and the heavyest armor penetration you could ever imagine.
Tons of damage, stun, tons of armor pen (cause the Cleaver and cause your skill), tons of allied damage, and tons of healing (cause the vamp, and cause the skill).
Its a hard hit. But slow to reach.
