Kass, Tryndamere, Rammus, Morgana, Sion, Shaco in order of importance for me.
When I play against Sion mid (I usually play TF) I just kinda ignore him and pop his shield. He then can't kill you / push lane up if you're popping his shield. I also have a great ability to gank other lanes and not waste my time trying to kill him, so I feel like TF is a good counter to Sion. Probably another mid champ with good sustain that can push back when sion pushes the lane would be good as well.
Rammus's ganks scare me, and I've seen him carrying games lately so been banning him.
For the others not really sure. Haven't played against them much / laned against them.

Most people say BANBANBAN XXX champion because they are personally afraid of them and don't know how to counter them. Such as if someone is screaming "BAN KATARINA, SO OP!!!" My response will be, "Stfu and get somebody with a stun/silence. Use it when she ults, before our team is ripped apart."
Pay attention to the most recent patch notes and take into account the allies opinions as well as your own. Just don't ban somebody that you can easily counter, wasting that choice. It could cost you somebody else that will indefinately determine the game if you aren't careful. Such as Udyr... or Dufflaf :P
Trynd counter is Anivia. She slows him, he has to kill her twice, then he can't run if she uses her wall correctly. Woot.
I have found Malphite to be a good Singed counter with stealing his movement speed. Just don't chase him and he has extremely pitiful damage output.
Good counter to Riven would be Udyr. Riven can stun and do all kinds of other fancy stuff (I love her), but Udyr just bear/tiger/bear/tiger... The only better spam stunner is Kennen or Jax (don't know how they are reworking him).
Udyr counter is difficult...Nunu works really good with the spam slow. Udyr can't get near you to be effective. Baiting works pretty good. "Yes, I am one squishy AP Carry. Yes, you may gladly chase me into this bush... BOOM!"
Counter-jungle Shaco. Chances are, he probably already has a couple of his boxes out and low mana by the time you get there. You could get Oracle too.

Well banning is really important, so making sure you are banning the right champions is also very important. You have to ban champions that are very hard to counter, and can easily carry a game on their own, if not stomped early on. Quickly asking your team who they want to play is a good idea, you will get an idea for who will be very hard to counter and who will be easy to counter.
As Animore said, don't listen to people who shout about banning champions who they personally don't know how to counter, like
Katarina or
Brand. As animore also said, tell them to pick a character with good CC and a stun, or a character with some decent power in mid,
Annie comes into mind.

Evelynn,
Twitch,
Dr. Mundo,
Katarina,
Swain and
Master Yi. They're all too hard to counter!
But no, you have to take in mind what composition you'll be making up - You'll want to ban people like
Kassadin and
Morgana, who work well against most opponents all the time. However, don't ban easily countered heroes -
Katarina requires a stun,
Evelynn requires an oracle,
Swain requires an
Ignite. Take these into mind when you're banning.
Back onto my point, you'll want to think what sort of composition your team leans to - If you have
Galio top,
Brand mid,
Graves and
Sona bot, and maybe
Nunu & Willump jungle or something, your team is probably leaning towards an AoE composition, meaning you'll want to ban counters to the composition - Tanky DPS with stuns or disables, and possibly gapclosers, people like
Udyr,
Irelia,
Olaf,
Alistar,
Amumu and many others come to mind, you'll want to ban the stronger or more common of these -
Amumu,
Udyr,
Irelia, possibly
Olaf if you find yourself against Dufftime.
Also, when banning, think about who the most important part of your team will be - Will it be your solo top

Vayne and
Kog'Maw. Will it be your
Kassadin mid? (What hacks did you use to manage to pick him?!) Ban Maokai or anyone else tanky with ranged disables, or another repeatedly usable jump. Think about what makes the character strong, and think of things that counter this mechanic - Like
Akali relies on using her stealth to survive, and her bursting to take down characters - People like
Lee Sin, who can see invisible targets, and survive the burst, or people like
Garen who can also survive burst, and deal damage whilst she's stealthed.
That's pretty much all I can be bothered to write at the moment.

Soraka isn't really a Karthus counter.
If they were banning Soraka, I'd say Urgot would be a good counter ban. Soraka completely shuts down Urgot's harassment.

Actually Soraka is a counter to Karthus (as well to a semi extent Urgot). I would say Sona is probably more of a counter to Urgot though.
Sion is a large counter to most AP mids. He especially rapes Annie.

I can't really say there is a hard counter I've run across for Kassadin, which is why he gets banned so often. The easiest soft-counter is to go with an auto-attack champ or someone like Ahri who can zone him early (her auto-attack range combined with the speed/range of her skillshots really does a number on slow, pre-blink Kass), but that only works to get him behind during laning phase, and if he can farm his way back in good luck stopping him.
