But why buy them on Teemo if you can get better items like Wit's End, Frozen Mallet or Wriggle's Lantern first. Hell, even Nashor's Tooth is better. Even on the champ, which would suit them best, they are not first pick.
And Kog'Maw doesn't build them anymore for a while now. On-hit just isn't viable. AD Kog all the way.
Finally, on which champ except Teemo they would be usefull? Also counting in how much Madred's Bloodrazor costs and how easy it gets countered.
And why people using them? Well, they are really big noob traps. All stats look so awsome first, until you exploit their weakneses. It is the same problem like Nashor's Tooth.

You fail to mention -
Malady's bad itemisation (no champion wants AP, MPen, and AS (together) in most circumstances)
That's the main reason people don't get it.
As for Madred's, I agree. Proc is too small, and it doesn't give you anything else. A better way to counter health stacking is to just get decent items and do damage that way.
Quick fix -
4% Magic Damage changed to 5% physical
AD from 40 to 55
Reduce recipe price to 550
